Notice of Privacy Practices
Last updated: August 2026
Our Commitment to Your Privacy
ClearStep Neuropsychology is committed to protecting the privacy of protected health information ("PHI"). We are required by law to maintain the privacy of PHI, to provide this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.
How We May Use and Disclose Health Information
- Treatment. We may use and disclose PHI to provide, coordinate, or manage evaluation and care, including sharing information with other providers involved in your child's care with appropriate authorization.
- Payment. We may use and disclose PHI to obtain payment for services, such as providing a superbill or documentation you request for insurance reimbursement.
- Health Care Operations. We may use PHI for necessary administrative and quality-related activities of the practice.
Uses and Disclosures Requiring Your Authorization
Most uses and disclosures of psychotherapy notes, uses for marketing, and any sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.
Other Permitted or Required Uses
We may use or disclose PHI without authorization when required or permitted by law — for example, to comply with legal or regulatory requirements, for public health and safety activities, to report suspected abuse or neglect, or in response to a valid court order.
Your Rights
- Request access to and a copy of PHI we maintain about you or your child.
- Request an amendment of PHI you believe is incorrect or incomplete.
- Request an accounting of certain disclosures.
- Request restrictions on certain uses and disclosures.
- Request confidential communications by alternative means or location.
- Receive a paper copy of this Notice upon request.
- File a complaint if you believe your privacy rights have been violated, without fear of retaliation.
Our Duties
We are required to maintain the privacy of PHI and to notify affected individuals following a breach of unsecured PHI. We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as information we receive in the future.
